  1. Complete Form W4-V (available here).

  2. Choose 7%, 10%, 12% or 22%, depending on your withholding needs. Check with us if you're unsure.

  3. Sign the form.

  4. Mail to your local SS office. Use this Social Security Office Locator to find the correct address.
